There is no charge for these replacement ID cards. Please furnish one clear COLOR CURRENT copy of either a state issued driver’s license with photo from Texas or another state; Texas identification card; government (U.S. and/or foreign) issued permanent residency card; passport; embassy card; identity card; etc, or other ID cards issued by a state or the federal government that display the person’s photograph, name, address, date of birth, physical description and signature. Birth certificates may not be used to verify birth date. Do not cut photo out.

These copies may be brought to initial registration appointment, mailed, or scanned and emailed to FWASA office, or brought to the FWASA office to be copied here. FWASA Office reserves the right to reject any copies that are not clear and legible. Do not cut photo out…BRING COPY INTACT. Please mark out ID number.

Please begin contacting your players now! Players may begin emailing or dropping off their above mentioned color copies to the FWASA office immediately. Please ask them to furnish their team name when they send in their copies.

Any players who were new last season and made new ID cards do not have to do it again in the Fall.

The easiest way to determine this is to look at the player ID number on the front of the card. If the last 4 digits are 0509, 0609, or 0709, the player does not have to replace their card. Player ID cards with any other numbers in the last 4 digits will need to be replaced.
